APT Notice of Nondiscrimination

APT is committed to ensuring equal access to care and services for all individuals. If you believe APT has failed to provide services or discriminated in another way, you may file a grievance with our compliance department at compliance@apt-management.com Grievances must be submitted in writing (via email) or through our anonymous hotline and will be promptly addressed through APT’s internal grievance procedure, available at 1-855-806-4296.
